2026-2027 School Year: Pre-participation Forms
Please note that our new Rank One online athletic registration system is still being finalized and is not yet ready for use. At this time, all student-athletes and their families should continue completing and submitting athletic paperwork using our current paper forms and existing process. If you have already turned in pages 5 and 6, just complete the other pages and turn those in and we will combine the packet. We appreciate your patience as we work to ensure a smooth transition to Rank One. Once the system is fully operational, we will notify families and provide instructions for completing future athletic registrations online.
